Registration
Prior registration is required. Space is limited.
To register, either email, fax, or mail the following information by December 28, 2007:
- Curriculum Vitae (that includes complete address, street address, telephone, fax and e-mail) or resume.
- Two (2) letters of support from the director of your institute or department, mentors, instructors, supervisors, or preceptors.
- One (1) page letter outlining why this practicum is important to you to your career development
You will be notified upon receipt of this information if you are accepted.
