Step 5: Closing Out a Cancer Prevention Clinical Trial Investigational Agent Accountability and Return By the time the closeout visit occurs any remaining study agent should be returned to the IND sponsor. The guidelines for agent returns (doc, 32kb) should be followed. Required documentation at the site includes: Receipt of return shipment Investigational agent log reflecting zero supply Binder containing pharmacy records, drug receipts and returns, drug log Accountability Form (Site Monitoring Manual) (pdf, 21kb) DCP Return Drug List Form (doc, 71kb) Sites must follow DCP Investigational Agent Accountability Guidelines.
